The Screaming Eagle Medical Home is the Army's first community-based primary care clinic. The clinic is located next to Gateway Medical Center in Clarksville in Medical Office Building One, 647 Dunlop Lane, Suite 301.

This new clinic operates on a patient-centered primary healthcare model using a team based, comprehensive approach to meet the patients' complete primary care and wellness needs. On-site services include primary care, pharmacy and laboratory services.

Enrollment is open to active duty TRICARE Prime Family members living in the following zip codes: 37010, 37032, 37035, 37040, 37043, 37146, 42204, 42234, and 42265. Also, active duty TRICARE Prime Family members who live in the 37042 zip code and are within a 10 minute drive time from the clinic are eligible to enroll. 

Eligible beneficiaries may choose to switch to the Screaming Eagle Medical Home or remain enrolled to their current healthcare provider.

Eligible beneficiaries who would like to change their enrollment to the Screaming Eagle Medical Home should complete and sign a Primary Care Manager (PCM) Change Request Form. The form may be downloaded here or obtained at the Tricare Service Center from 8 a.m. to 5 p.m., Monday through Friday. The Tricare Service Center is located in Bldg. 2525, 22nd and Indiana Ave. Beneficiaries who need assistance completing the PCM change request should contact 1-877-TRICARE (874-2273).

After the form is completed and signed, submit it through one of the following processes:

2) Fax to (931) 431-6168
3) Take the form to Fort Campbell’s Tricare Service Center during operating hours.
4) Mail to the Tricare Service Center, 2525 22nd St., P.O. Box 444, Fort Campbell, KY 42223

*** Patients should continue to be seen at their current clinic until they recieve confirmation from TRICARE that their assignment to the Screaming Eagle Medical Home is complete. After patients are successfully assigned to the Screaming Eagle Medical Home, they should continue to schedule or cancel appointments by calling 931-431-4677 or 270-798-4677.***
