One of the great benefits of the Patient-Centered Medical Home model is the improved continuity of care. For the model to work effectively for you, it is important that your care team be involved in your care. Therefore we ask that you reserve Emergency Room visits for emergencies.
If you have a non-emergency medical concern after hours, please send a message to your care team through www.RelayHealth.com. You may also contact the Call Center at (270) 798-4677 (HOSP) or (931) 431-HOSP Monday – Friday 6 a.m. - 6:30 p.m. to submit a telephone consultation (TCON) request.
While the Army Medicine Access to Care standards state that patient with urgent and acute concerns will be contacted within 24 business hours, the medical home staff works hard to contact you within two hours. Similarly, the Access to Care standard states that patients with routine concerns will be contacted within three business days, we make every effort to reach you within 24 business hours.
The medical home team makes every effort to contact patients with urgent and acute concerns within two hours, but will contact you within 24 business hours. In addition, we work hard to contact patients with more routine concerns by the end of the business day but will contact you within three business days.
If you feel that your condition is an emergency, please visit the nearest Emergency Room.
Because your continuity of care is so important, your care team will follow up with you following an emergency visit. This ensures the emergency or urgent medical concern is taken into consideration with your care plan.
